Schedule:

• Monday to Friday, 8:30 AM - 5:00 PM

Responsibilities:

• Providing hospice social work services

• Evaluating patient care needs and developing plans to improve quality of life

• Communicating with the home health and hospice care team

• Maintaining accurate assessment and progress documentation

Additional Information:

• 13-week assignment starting immediately

• Minimum of 2 years of recent experience in home health, hospice, or palliative care preferred

• Required certifications: MSW (Master's in Social Work), CPR, BLS; LCSW considered if the candidate has a California license

• Patient population: Adults, primarily ages 50 and older, with a caseload of 4-5 patients per day
