Requirements:
• Valid Arizona Physical Therapist license
• Minimum of 1 year of experience in a clinical setting
Schedule:
• 13-week assignment
• Start date: Immediately
• 8a - 5p shifts from Mondays - Fridays
Responsibilities:
• Provide physical therapy services to residents
• Conduct evaluations and develop treatment plans
• Treat patients with high-quality practices and care
• Perform chart reviews and document accurately
Additional Information:
• Nursing facility with a staff mix of PT, COTA, PTA, OT, and SLP
• Patient population includes adults aged 18 and up
• Daily patient load of 10-12
• EMR systems: NetHealth & PCC
