Job Description

Job Title: Hospice/Palliative Care Nurse

Job Location: California

Type of Facility: Home Health and Hospice

Length of Assignment: 7 weeks

Start Date: Immediately

Work Schedule: 8:30 AM - 5:00 PM shifts

Roles and Responsibilities:

- Support Nurse Case Manager in patient care

- Communicate effectively with Team Leaders, HHAs, and other healthcare providers

- Instruct caregivers, patients, volunteers, and employed caregivers based on the Plan of Care

- Perform nursing procedures as requested

- Travel up to 40-60 miles per day (mileage reimbursement provided)

- Provide care for 4-5 patients per day, primarily older adults 

Requirements:

- Bachelor's or Associate's degree in Nursing

- Active California RN license

- BLS certification through Red Cross or American Heart Association

- At least one year of recent hospice experience (within the last year)

- Proficiency in Chinese languages strongly preferred

- Experience with Oasis and Epic EMR systems is a plus
