Job Description

Job Title: Home Health Nurse

Job Location: Tennessee

Type of Facility: Home Health Agency

Length of Assignment: 4-8 weeks

Start Date: Immediately

Work Schedule: Monday-Friday, 8:00 AM - 4:30 PM

Roles and Responsibilities:

- Provide home health care to 3-6 patients per day, primarily geriatric population

- Perform starter care, discharge, and regular visits

- Conduct wound care, manage short-term IVs, and treat Stage 3-4 ulcers

- Draw labs, manage catheters, and apply leg/compression wraps

- Complete thorough and accurate documentation using OASIS assessments

Requirements:

- Active RN license (compact license accepted)

- ASN or BSN degree

- BLS certification

- Proven home health experience

- Proficiency with EMR systems, particularly point of care/Homecare home base
